Model Release Form

Consent:

I, the undersigned person, hereby grant permission to Twenty10 Photography and the photographer, to photograph me or anybody associated with my booking and to use the images and any reproductions or adaptations of the images for all general purposes, in any form of media, including but not limited to advertising, marketing, social media, publications, and other promotional uses. This release applies worldwide and in perpetuity.(Required)

Parent/Guardian Consent (If Model under 18):

I, the undersigned, am the parent, or legal guardian, or the representative who has the relevent concent forms to allow photographs of the minor, or minors taking part in this photoshoot. I have the legal right to consent to and I do consent to the terms and conditions of this model release.

Use of Images:

The images may be used in both print and digital formats. They may be edited, altered, or combined with other images, graphics, text, and other media. The photographer reserves the right to use the images for portfolio purposes, promotional materials, product sale purposes or for any other lawful purpose.(Required)

Compensation:

I understand that I will not receive any monetary compensation for the use of the images. The consideration for this release is the opportunity for the modeling session and any agreed-upon benefits, such as prints or digital copies. I will also have access to purchase any images made available for reproduction(Required)

Rights and Ownership:

I acknowledge that the images, along with any associated copyright, are the sole property of Twenty10 Photography. I waive any right to inspect or approve the final product or the use to which the images may be applied.(Required)

Release from Liability:

I release Twenty10 Photography, their legal representatives, and assigns from all claims and liability relating to the images. This includes any claims for libel, slander, invasion of privacy, or other tortious acts.(Required)

Acknowledgment:

I am of legal age and have the full legal capacity to execute this release. I have read this release before signing and fully understand its contents, meaning, and impact.(Required)
